Expected Outcomes
Our multidisciplinary team, led by a rehabilitation physician, works with patients and their families to achieve the following goals:
improve mobility through training in walking, assistive devices and transfer techniques. improve self-care skills in bathing, dressing and feeding. increase knowledge about the recovery and rehabilitation process. establish independence in bowel and bladder function. develop behavior management skills. maximize verbal communication and resume normal swallowing function. attain highest level of cognitive function possible. establish a plan for nutritional health.Patient Profile

Our approach to care includes:
physical therapy to improve mobility. occupational therapy to improve self-care skills and enhance independent living. cognitive rehabilitation to re-establish the ability to process and communicate information. speech therapy to enhance the patients ability to speak, understand and swallow/eat safely. psychosocial support to help patients and their families adjust to the changes in daily living. rehabilitation nursing to educate about bowel and bladder function, sexuality and skin management. patient education on diabetic care and secondary health issues. nutritional counseling to help promote a healthy lifestyle.Continuum of Care
From preadmission to community re-entry, our program is designed to meet the individual needs of patients and their families. Care is available on an acute and postacute inpatient and outpatient basis. We also provide these specialized services:
mobility court. transitional living apartment. therapeutic pool. vocational rehabilitation. community reintegration. referral service for appropriate resources and support systems. onsite support groups. home evaluations to ensure a safe transition.
